Membership Application
In order to apply to be an Active Member, please download the forms below.
The Membership application is for the Lodge. All Active Members must be full time State Law Enforcement Officers, full time Law Enforcement serving in the State of Oklahoma, or Retired State Law Enforcement Officers.
For State Employees, dues may be paid either monthly through payroll deduction to your FOP Credit Union Account, automatic drafts from your bank account, or annually/monthly by the member. Payroll deductions are sent to the Member's account at the FOP Credit Union in Tulsa. If a new member chooses monthly payroll deduction, please download the New Member Credit Union Application and Payroll Deduction Form.
Complete the Application documents and mail them along with a copy of your driver's license (the credit union requires it) and a check for $45.00 for the first month's dues and credit union fee. to FOP Lodge 219 3429 E Trenton St., Broken Arrow, OK 74014. All forms must be sent to the Secretary at the Lodge address so the Secretary can forward the forms where they need to go and enroll the member.
Once a member is enrolled in the Lodge, monthly dues will begin being deducted from payroll. Membership is effective as soon as the Lodge Secretary completes the enrollment.
The Emergency Contact form is not required but the Lodge tried to keep track of where to find your family in the event of an emergency where a member's family might need FOP assistance.
