Membership Application
In order to apply to be an Active Member, please download the forms below.
The Membership application is for the Lodge. All Active Members must be full time State Law Enforcement Officers, full time Law Enforcement serving in the State of Oklahoma, or Retired State Law Enforcement Officers.
Dues may be paid monthly, quarterly or annually through automatic drafts from your bank account, by personal check, or your bank's bill pay system.
The completed Application for Membership and Emergency Contacts form can be sent to the Secretary by email at vtl0401@yahoo.com but a check for the first month's dues of $40.00 is needed to complete the application process. Please also include either a copy of your agency credentials or CLEET card to show the applicant is an LEO i Oklahoma. The Lodge address is FOP Lodge 219 3429 E Trenton St., Broken Arrow, OK 74014. All forms must be sent to the Secretary at the Lodge address. Membership is effective as soon as the Lodge Secretary completes the enrollment.
The Emergency Contact form is not required but the Lodge tried to keep track of where to find your family in the event of an emergency where a member's family might need FOP assistance. Members are also eligible for an account at the Tulsa FOP Federal Credit Union. The cost if $5.00 to join. The CU application below can be completed and sent to the Secretary along with the CU application fee of $5.00.
