New Member Application
Apply as a new active member if you are a full-time state law enforcement officer, a full-time law enforcement personnel serving in the state of Oklahoma, or a retired state law enforcement officer. The application is for lodge membership, and dues may be paid monthly, quarterly, or annually through automatic drafts from your bank account, by personal check, or through your bank’s bill pay system.
Application Process
The completed Application for Membership form can be submitted to the lodge secretary by email at vtl0401@yahoo.com. A check for the first month’s dues of $40.00 is also required to completely process the member application request. or use an Automatic ACH to have dues drafted monthly. Additionally, please submit either a copy of agency credentials or a CLEET card to show the applicant is a legitimate Oklahoma law enforcement officer.
All documents must be either emailed to the Lodge Secretary at vtl0401@yahoo.com or mailed to the address below:

While this form is not required, we strive to keep track of how to contact your family in case of an emergency or if a family member needs immediate FOP assistance. If interested, please fill out the CU application form below and send it to the lodge secretary along with the CU joining fee.

REQUEST MEMBERSHIP IN THE TULSA FOP FEDERAL CREDIT UNION
All FOP Members are eligible to join the Tulsa FOP Federal Credit Union. Fraternal Order of Police FCU. There is a one-time $5.00 membership fee. If you are interested in joining the credit union, just complete the attached Credit Union Application and mail it to the Lodge Secretary along with a check for $5.00 or mail it directly to the Tulsa FOPFCU at the address on their website.
